This probably has something to do with how I generate the .t3d data. I reverse engineered the format for the most part, so I could have missed something or gotten it wrong.

MMC was primarly designed as a guide to make building the maps (in Quake 3 incidentally) by hand easier. When I came across the M:R effort I added the .t3d support, which turned out to be easier than supporting Quake maps.

Unreal maps are inside-out in the sense that the world is solid and you add empty spaces to form rooms. This makes things easy since Marathon maps are inside out too; Quake maps need walls and floors and ceilings.
