In the investigation I did on that matter I have found UT identifies the buttons as: left, middle and right. There is no 1, 2, 3, 4... etc.

However, I have just found that: http://www.macgamer.com/resources/ut/mousesettings/mousesettings.chtml

It says (I can't try it now, I'm at school, and I don't remember from when I tried this) that the extra button should register as joystick buttons. But it will only work if you bind the command to a key and THEN to the button. The article is on Macs though. I'll try this at 15h30 when I get home.

My mouse is an MS Optical mouse, what's yours? I don't think it's mouse related, but just to know...
