Assuming you're using Windows... when you install UT, UT\System\setup.exe is registered as the handler for files whose name ends in ".umod". Often tho, Windows hiccups and forgets this fact. If this happens then run setup.exe yourself, passing "install" as the first parameter, and the path to the .umod file as the second.
Example: Start | Run | d:\games\ut\system\setup.exe install "d:\downloads\MarathonResurrectionb1_2.umod"
The installer will do the rest.
