What takes longest is usually subjective, and things can drag out for unusual long times for a number of arbitrary reasons (such as what is good enough, countless improvements etc). Also, something takes longest when you have the least number of skilled people working on it, or when there are simply giants amount of something to do. In M:Rs case, creating the levels "seems" to take longest because there are so many of them, and because they require elements from all other aspects of the game (models, animation, sounds, lighting, sprites, code, scenery etc).
In short, there's no straight answer. Every individual bit is a large or small a task as you make it to be. Generally speaking though, if you want something done very well then EACH aspect from modeling to texturing to level design will take about the same amount of time...LOTS.

The crosshair option in M:R is normally disabled. Due to some demand, the crosshairs have been implemented in the multiplayer part of the game as a serverside option. If you join a hosted multiplayer game that has this option turned on, or if you create a game in which you turn this option on for multiplayer you will see crosshairs, otherwise..you won't. There are no crosshairs for the single player game.

The demo (version 1.2) shouldn't have this problem. I recommend removing all bits of previous version of M:R you might have had before and re-install M:R 1.2 again. If the problem persists, then it is a legitimate bug that has been dormat from previous versions.....hmm.
