I'm pretty sure I did. That would be a reasonable mission parameter for this level, but I don't remember if that's the way it was in original Marathon*. I do know, though, that mission parameters can't affect whether a switch work or not in original Marathon. By getting to that switch, the player has pretty much been exposed to all areas of the map already. From what I can tell, it looks like there's a bug with the switch.

*Okay, I just tried the level in M1A1 (playing original M1 would be difficult or impossible for me). When I hit the switch in M1A1, it opened the door immediately. I was also able to then walk to the starting area and teleport to the next level *despite* purposefully leaving some aliens alive. So unless M1A1 missed the extermination mission parameter, that's not a requirement of the original level.
