After some review, I've determined that this is probably a glitch with the way UnrealEd does CSG with the brushes in the map. Although the two joining faces appear to be coplanar there may be some rounding errors within UnrealEd, causing the small face to appear.

I've tried a number of tricks and have found that switching the order of the two offending brushes (Brush158 and Brush167) within the map effectively tricks UnrealEd's CSG to behave properly.

Unfortunately UnrealEd is, from a technical perspective, a pile of garbage. A lot of things are completely backward from how they should have been implemented, simply because the programmers who wrote UnrealEd were used to using a backward language.
